Speech Therapy Coding Changes Coming January 1, 2027
Starting January 1, 2027, Current Procedural Terminology® (CPT) code 92507 will be replaced with 10 new time-based CPT codes. Today, CPT code 92507 is used for individual treatment of speech, language, voice, communication, and auditory processing disorders.
Why it Matters
The new codes will cover 5 treatment categories. Each category will have a base code and an add-on code for additional treatment time:
- Fluency disorders
- Speech sound production disorders
- Language comprehension and expression disorders
- Combined speech sound production and language comprehension and expression disorders
- Voice, upper airway dysfunction, and/or resonance disorders
What’s Changing
You’ll bill speech therapy services using the appropriate time-based CPT code based on the service provided and the time spent delivering direct, one-on-one patient care.
- Report only the time spent providing direct care that’s documented in the patient’s medical record.
- Don’t count the same treatment time toward more than 1 service.
- If you report more than 1 code from the new code range, each service must be separated and supported by the medical record.
- Don’t bill the speech sound production code and the language code together. Use the combined speech and language code instead.
What Isn’t Changing
The CPT code 92508 for group treatment of 2 or more patients will not change. This will continue to be reported as an untimed code.

- Review 2027 coding guidance from the American Speech-Language-Hearing Association.
- Assess your billing, coding, documentation, and operational processes for the transition to time-based billing.
- Watch for additional education, billing guidance, and transition resources from Premera Blue Cross Blue Shield of Alaska and the Centers for Medicare & Medicaid Services (CMS).
The American Medical Association has not yet released the official 2027 CPT codes, and CMS has not finalized its 2027 rule. We’ll share more information when official guidance is available.
