Drug List and Medical Policy Changes Coming

What’s Changing

As of January 1, 2026, some drugs move to a new formulary list. The following drugs will also require a new prior authorization:

Autoimmune conditions

  • Bkemv (eculizumab-aeeb)
  • Soliris (eculizumab)
  • Epysqli (eculizumab-aagh)

Inflammatory conditions

  • Cyltezo (adalimumab-adbm)
  • Adalimumab-aaty
  • Adalimumab-adaz
  • Adalimumab-adbm
  • Adalimumab-ryvk
  • Simlandi (adalimumab-ryvk)
  • Yuflyma (adalimumab-aaty)
  • New prescriptions: For prescriptions dispensed on or after January 1, 2026, submit a prior authorization.
  • Existing prescriptions: If a preferred alternative isn’t right for a patient already taking these prescriptions, submit a prior authorization. Approved requests may cost more than preferred drugs, depending on the member’s health plan.
