What’s Changing
As of January 1, 2026, some drugs move to a new formulary list. The following drugs will also require a new prior authorization:

- Bkemv (eculizumab-aeeb)
- Soliris (eculizumab)
- Epysqli (eculizumab-aagh)
Inflammatory conditions
- Cyltezo (adalimumab-adbm)
- Adalimumab-aaty
- Adalimumab-adaz
- Adalimumab-adbm
- Adalimumab-ryvk
- Simlandi (adalimumab-ryvk)
- Yuflyma (adalimumab-aaty)
- New prescriptions: For prescriptions dispensed on or after January 1, 2026, submit a prior authorization.
- Existing prescriptions: If a preferred alternative isn’t right for a patient already taking these prescriptions, submit a prior authorization. Approved requests may cost more than preferred drugs, depending on the member’s health plan.
