Better Care Through Population Health

Population health at Premera Blue Cross Blue Shield of Alaska is guided by the National Committee for Quality Assurance (NCQA). Our goal is to improve experience, affordability, and health outcomes for targeted groups. We do this by strengthening care evaluation and community health programs.

What We’re Doing

• Listening to patients through a member experience survey for the fecal immunochemical test (FIT) kit program.
• Improving care transitions by increasing the number of patients who complete an outpatient follow-up visit after a hospital stay.
• Enhancing patient experience for those enrolled in complex case management.
• Expanding diabetes support by increasing participation in the digital diabetes management program.

Keep in mind that some Premera members may not be eligible for population health programs.
