Appeals Fax Number Change

Premera Blue Cross Blue Shield of Alaska uses one appeals fax number: 425-918-5592. This number is noted on all appeal forms. Be sure to make a note of the current number in your provider’s office. Remove the former fax numbers (425-918-4133 and 800-557-7581).

When submitting an appeal on behalf of a member, make sure to use the correct form and get member authorization before submitting the appeal.
